Every
order placed with CDFX requires the following documentation:
- Indemnification Form ( .doc

- This form is to be printed on
company letterhead, signed and faxed or
emailed to CDFX. No order will be
processed until all documentation

- Master Services Agreement (.pdf )
- This agreement is to be
printed, signed and faxed or emailed to CDFX.

- This form is to be
printed, signed and faxed or emailed to CDFX.

CD Wizard ordering
system
All of the ordering on CDFX is done through the
CD Wizard. To order any of our products
as well as access our CD face graphic art
templates simply log in and peruse our wide
variety of products, services and purchase
options. If your purchase involves replication
and you have found what you are looking for, the
CD Wizard will guide you through the
ordering process listed below:
1) Have all art templates and CD content ready
2) Select
items for purchase
3)
Checkout
4)
Upload files
5)
Receive order confirmation number
Once you have
completed the above steps you will also receive an automatic email
receipt. All order tracking information is
available online at anytime so you always
know what the
status of your project is at anytime.
Even though CDFX prides itself on the ability to
provide a complete automated solution
we also understand that problems and questions
sometimes arise. It is for this reason
that we have representatives available to you
Monday - Friday, 6:00 a.m. to 8:00 p.m.
